Photo Release Form

All students need to turn in photo release forms (if you did not fill one out at Registration). The form is here. Without this form, your photos cannot be posted on the band website.

If you do not choose to sign the release, you should send an email to that effect to Mr. Nuñez so that he can inform the Booster photography team and the Webmaster.

If you choose to not allow your student to be photographed then no photos of that student will be taken at all during the year under any circumstances by the Booster photographers (accidental images will be deleted), and the student will not appear in the Banquet video nor the Yearbook DVD collection.
